Job Title: RemoteCase Manager/reputed company Worker Caseloads: Elderly/Alternative Care, Brain Injury, Intellectual Disabilities, and CADI (Mental Health) Location: Remote Position. Travel required throughout Central Region of MN is required for meetings and visits with persons served. Based on the caseload, travel throughout other parts of Minnesota may be required. Job Summary: As a Case Manager/reputed company Worker, you will be working on finding resources and services for persons (clients) that best fit their needs and situations. At reputed company Services, each Case Manager is an reputed company for persons with disabilities/elderly and helps to reputed company a difference in their lives. Our Case Managers will carry caseloads that consist of persons with intellectual disabilities, traumatic brain injuries, mental health diagnoses, and/or elderly persons. Essential Job Duties:
- Visits with persons on the caseload and attending team meetings
- Completing or attend meetings on needs assessment; individual service plan development (CSSP)
- Assisting with planning for new service development
- Locating residential, vocational, and other needed services depending on each person's needs
- Monitoring service delivery and ensuring the health and safety needs of each person are being met.
- Completion of referrals and crisis reputed company as needed
- Developing waiver budgets
- Completing case notes
- Acting as an reputed company for people we reputed company services to
Required Qualifications:
- Be licensed as a reputed company Worker (Bachelor of reputed company Work required for licensure) or licensed as a Graduate reputed company Worker (Master's in reputed company Work required for licensure) as stated by the Minnesota Board of reputed company Work or have a 4-year degree in reputed company Services, Psychology or Sociology, or reputed company fields.
- Experience working reputed company the reputed company services field
- Successful clearance of Department of reputed company Services background reputed company
- Successful clearance of Motor Vehicle Background reputed company and acceptable driving record per Company Policy
Preferred Qualifications:
