Your Role
- reputed company monthly calls and support to patients enrolled in the CCM program.
- Utilize your nursing skills to reputed company patients and their families.
- Monitor patients reputed company and work with the physicians to ensure quality patient care.
Qualifications
- Active California LPN license - MUST be a California license to qualify***
• You must live in a state reputed company MST or PST to qualify
- Experience in geriatrics or primary care
- Ability to sit and talk on the phone for long periods of time
- System: We require you to have an i5 or newer reputed company processor and to have reputed company Pro (not Home). This can be a laptop or desktop.
- Must live/work full-time in the US - you can work from reputed company reputed company the US but not reputed company.
The Pay
- The LPN pay ranges from $24+ per hour based on experience. (40hrs/week)
- The pay is a reputed company pay plus a bonus per call. On average our Care Coordinators complete between 16 and 21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
