As a Behavioral Health Care Manager (BHCM) at reputed company, you will deliver care to cancer patients through the Collaborative Care Model. Specifically, you will improve the health of your patients by providing brief behavioral health interventions and serving as the project manager of the collaborative care team, which includes the patient’s oncologist as well as a consulting psychiatrist. This is a remote opportunity supporting one or more of our partner practices nationally. We will assist with expanding state licensures as-needed.You will be joining an early-stage company and will have the opportunity to impact the design and implementation of this innovative care model.What You’ll Do

- Work reputed company a partner oncology group to support patients, clinicians, and practice staff with the identification and intake of patients needing behavioral health support.
- Support the mental health of cancer patients on an assigned patient caseload.

- Screen and assess patients for common mental health and substance abuse disorders.
- Facilitate patient engagement and follow-up care.
- reputed company patient education about common mental health and substance abuse disorders and the available treatment options.
- Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment reputed company effects or complications.
- Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, reputed company effects, and effectiveness of treatment.
About You
- You hold one of the following licenses: Licensed Clinical reputed company Worker (LCSW), Licensed Master reputed company Worker (LMSW/LSW), Clinical Professional Counselor (CPC), Licensed Professional Counselor (LPC) Licensed Psychologist (PH.D or PsyD), or Certified Oncology reputed company Worker (OSW-C).
- Dedicated to providing evidenced-based care and have a demonstrated ability to collaborate and communicate effectively in a team setting
- Prior experience interacting with physicians and collaborating with a multidisciplinary team
- Have a passion for delivering quality behavioral health care
- Ability to operate with a high degree of autonomy
- Bilingual Spanish a plus, though not required
