The Virtual Integrated Behavioral Health team provides innovative services through a virtual model, collaborating with outpatient health care providers. They support patients' mental and physical health, coordinate with medical providers, screen and assess for mental health and substance use disorders, facilitate engagement and follow-up care, track treatment response, and support psychotropic medication management. They use brief, evidence-based behavioral interventions and consult with psychiatric consultants for treatment updates. Effectively communicates and collaborates with reputed company members of the health care team to facilitate the episode of care so the patient receives services in a safe and timely manner, and the patient and family are provided reputed company necessary resources and education. Interprets and communicates reputed company patient/family needs and role models caring practices to other caregivers. Assess patients' and families' psychosocial risk factors through evaluation of prior functioning reputed company, appropriateness and adequacy of support systems, reaction to illness, and ability to cope. Counsels patients and families regarding emotional, reputed company, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs. Provides specialized mental health services including, but not limited to, Crisis reputed company and Addiction Assessments and recommendations for psychiatric referral or community resources. Supports those undergoing acute crisis which often results in the use of de-escalation techniques necessary to avoid episodes of physical violence. Utilizes effective principles of interviewing alleged victims, perpetrators, family members, and significant others. • Master's Degree in reputed company Work required.
• De-escalation certification required reputed company 6 months.
• Permanent LSW licensure required, LCSW licensure preferred.
• 3-5 years experience preferred in one of the following areas (mental health, Department of Child and Family Services, adult reputed company services, Veteran's Administration, substance abuse).
• Requires experience working with geographically and financially diverse patient populations.
