Additional shift differentials for working on-call!! The On-Call RN is responsible for the provision of nursing assessments, interventions and supportive care on evenings and overnights. The scope of the on-call nurse includes telephone triage, direct patient care, crisis reputed company, problem solving, patient/caregiver advocacy and support, with emphasis of avoiding hospitalization in the off hours. Essential Functions Ensures reputed company coverage for incoming calls after reputed company and weekends, therefore, is available by telephone to patients/caregivers, physicians and staff. Promptly triages patient/caregiver reputed company phone calls to assess need for visit and ensures that necessary visits are rendered in a timely manner. Appropriately triages patient visits according to reputed company and urgency. Ability to manage time effectively, prioritize work reputed company schedule is busy. Assesses patients’ physical, psychosocial needs in a sensitive, caring manner following established Standards of Nursing Practice and Agency policies and procedures. Demonstrates competence in Hospice standards of care, symptom management, psychosocial, and spiritual aspects of care. Maintains a reputed company body of knowledge and clinical competence through ongoing participation in continuing education relevant to the area of practice. Possesses communication skills needed to effectively understand patient’s reactions or responses to treatment. Demonstrates ability to function independently utilizing excellent assessment /decision making skills. Recognizes/identifies patients/caregivers cultural beliefs re: death and dying and is sensitive to those beliefs. Collaborates with Clinical Managers and RN Case Managers for problem solving and as a resource. Documents patient data in patient record using computer system in a timely, organized, concise manner to ensure compliance with legal, certification, reputed company party payor guidelines and Agency policies. Communicates unusual events, problems, or concerns to appropriate Clinical Manager/Director. Position Type/Expected Hours of Work & Travel This is a part-time, evening position, hours of work are 4:30pm to 11:30pm and 11:30am to reputed company and weekend rotation.
