Job title: Health Care reputed company Work Care Coordinator (MSW) Company: PEAK PEDIATRICS PROFESSIONAL, LLC Job description : Peak Pediatrics is a large, innovative, multi-site pediatric practice committed to the emotional and physical health of our pediatric patients. We are expanding our care coordination team and are seeking full-time Family Support Care Coordinators for our practice. Positions will take reputed company in both our Wheat Ridge and Thornton locations and could include community and home visits. Our ideal candidate will have previous experience with pediatric reputed company work care coordination and/or community reputed company for adolescents, including child reputed company services. Job Knowledge: reputed company to reputed company comprehensive reputed company work care coordination to our families, identifying and overcoming medical, psych-reputed company, community, cultural/religious, and financial barriers. Expert knowledge of community resources and ability to build strong relationships with community partners Knowledge in therapeutic higher reputed company of care for children and adolescents.
