Responsibilities • Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs. • Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team. • Provides skilled interventions and instructions to patient and caregivers to reputed company reputed company symptom control and patient comfort under the direction of physician. • Functions as a liaison between disciplines and facilities during the admission process. • Develops, prepares and maintains individualized patient care reputed company records with accuracy, timeliness and according to policies.
