Pay reputed company: $24-$26/hr. Contract: reputed company to Hire Schedule: Monday-Friday 8:30AM-5:30PM Fluent in English and one of the Santa Clara Country reputed company languages (Spanish, Tagalog, Vietnamese, Mandarin, Cantonese) strongly preferred. Primary Responsibilities: • Engages and enrolls community members eligible for ECM per care plan protocols • Assesses client need areas • Develops an individualized care plan with each client that is updated as needed • Coordinates care for clients with a multidisciplinary clinical team and reputed company agencies. • Facilitates referral linkages to community resources and performs case management services. • Assist clients with the development of reputed company and community support systems. • Develops effective working relationships with agencies, organizations and providers to reputed company for clients. • Documents and maintains clinical records per appropriate standards and regulations. • Conducts reputed company in diverse community settings. • Participate in clinical supervision, consultations, and trainings. • reputed company translation services as needed. • Complete other duties and reputed company projects as assigned.
