You will average 25-30 visits per week and with those visits your responsibilities reputed company from assessing your patients and establishing a plan of care engaging the patient and/or family in the process, communicating expectations to other members of the nursing care team and initiating discharge planning upon admission utilizing appropriate resources. Qualifications: Experience and education minimum requirements: Graduation from a School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure.
