Key Responsibilities • Accurately review and code medical records and encounters for diagnoses and procedures, adhering to Risk Adjustment and HCC coding standards. • Implement ICD-10-CM, CMS-HCC, and other pertinent coding regulations to ensure compliance and precision. • Ensure the completeness, accuracy, and reputed company of coded data. • Address and resolve any discrepancies between clinical documentation and diagnosis coding. • Stay abreast of the latest coding guidelines, regulations, and updates reputed company to Risk Adjustment and HCC coding. • Uphold HIPAA compliance and maintain patient confidentiality and data reputed company. • Collaborate effectively with reputed company providers and team members to clarify documentation and resolve coding inquiries. • Engage in ongoing education and training to enhance coding proficiency and effectiveness. • Generate reports on coding activities, accuracy, and trends. • Support coding audits (both internal and external) to ensure adherence to coding practices. • Identify and recommend enhancements to the coding process and documentation accuracy.
