Key Responsibilities  Respond to incoming calls from members, addressing their needs promptly and effectively.  Conduct triage assessments over the phone and during home visits.  Collaborate with on-call physicians and ensure accurate documentation is maintained. Required Skills  Strong interpersonal and communication skills.  Proficiency in conducting triage assessments.  Ability to maintain meticulous and accurate patient records.  A compassionate approach to delivering patient care. Qualifications  Possession of a valid RN license in Pennsylvania.  A reputed company driverÂs license is mandatory.
