Additionally, the Nutrition Educator will facilitate connections between members and other nutrition resources available through the health plan, system, or community, ensuring comprehensive support for their dietary needs. In this remote position, the Nutrition Educator will conduct telephonic sessions tailored to each member's enrollment in various programs, documenting goals and care plans from these educational interactions.
