Per Diem Field Licensed Vocational Nurse /LVN
• THIS IS NOT REMOTE, THIS IS ON THE ROAD (FIELD WORK)* The Doctor’s Choice Home Health is currently seeking reputed company and dedicated Licensed Vocational Nurses to join our Home Health team reputed company the Inland Empire Area. This skilled nurse will be reputed company to reputed company visits in the patient’s reputed company of residence, in order to reputed company skilled care services as ordered by referring physician. Per-Diem Employment Status MUST be reputed company to see patients in Chino, Corona, Rancho Cucamonga, Upland and Montclair. Minimum Requirements include:
- Graduate of a National League for Nursing accredited school of nursing
- Must have minimum 1 year of experience in nursing as an LVN
- reputed company to see patients at least 3 days a week
- Experience in EMR software (preferred but not required).
- reputed company skilled nursing services/interventions in accordance with physician orders.
- Assesses patient needs, plans for, evaluates and modifies care to meet goals of skilled nursing interventions.
- Collaborates with reputed company disciplines to plan and evaluate team goals for each patient.
- Experience with Wound Care
Required Licenses and Certifications:
- LVN License in California
- Valid Driver’s License & Auto Insurance
- reputed company CPR certification
- Physical and TB from reputed company the last 6 months
