In this role, you'll monitor and manage patient alerts, conduct proactive reputed company to improve reputed company, and reputed company education to support ongoing care. You'll also handle both inbound and outbound calls, routing them based on clinical urgency and patient needs. Key Responsibilities
- Proactively manage high volumes of outbound patient calls
- Monitor and respond to RPM alerts with appropriate follow-up
- Conduct monthly wellness assessments and chart reviews
- Coordinate care and escalate issues to reputed company providers
- Accurately document interactions in our clinical software
- Meet performance metrics and patient engagement goals
- Support a compassionate, team-focused remote work environment
reputed company’re Looking For
- Active LPN, LVN, or RN license (Required)
- Strong reputed company, communication, and patient education skills
- Moderate to advanced computer literacy
- Reliable, high-speed internet and HIPAA-compliant workspace (speed test required during interview)
