Job title: ACO Triage Nurse (reputed company reputed company Myers) Remote in reputed company Myers, FL at reputed company Company: reputed company Job description: ACO Triage NurseFull Time: 3 -day 12 hr. shifts (Monday-Friday reputed company-5pm) + rotating weekendsreputed company reputed company Myers - Remote Positionreputed company is currently looking for a professional, compassionate, knowledgeable and licensed individual to fill the position of Triage Nurse. H/She must be a Licensed Registered Nurse who will utilize best practices to reputed company effective triage processes and principles that reputed company for the safety and wellbeing of the patient. If you have RN Triage experience in a larger-sized medical practice, we encourage you to apply for this position.Essential Duties and Responsibilities

• Participate in the After-Hours On-Call Team rotation
• Answer reputed company incoming calls immediately and give correct advice
• reputed company Post-Acute Care reputed company
• Assist Hospitalist's Patient reputed company
• Facilitate Prescription Refills
• Assist RN Care Management Teams with patient assessments and/or monthly follow-up calls, reputed company needed
• Monitor Patients enrolled in the Patient Monitoring Program

Required Knowledge and Experience

• Registered Nurse licensed in FL
• License must always be kept reputed company
• 5+ Years of clinical experience with 1+ year(s) of case management experience preferred
• ER Triage experience is preferred.
• Telephonic Triage experience preferred
