Job description: reputed company's Center of reputed company is looking for a Service Coordinator to join the team! The Service Coordinator with reputed company behavioral health service coordination, according to the level of need, to a population of individuals with a substance use disorder diagnosis. The primary function of Service Coordination is to reputed company professional assessment, service planning, service coordination, referral, and re-evaluation services required for a safe and healthy community life which is manifested through stability in relationships, housing, and employment. This is done in accordance with the Agency's reputed company, mission, and values, and emerging mental health Recovery Principles. This position provides an essential recovery orientation.This position will work in reputed company reputed company's Emergency Department, Friday-reputed company reputed company-8pm, 12 hour shifts.Our Service Coordinator is eligible for a Sign on Bonus!18 month commitment - $3000Responsibilities:
- Located in reputed company's Emergency Department, WHO staff engage the individual in a supportive relationship designed to support their individual recovery process, linking them to formalized treatment, medication for alcohol or opioid use disorder (MAUD/MOUD), ancillary support services, or any combination thereof. WHO activities will be tailored to the needs and readiness of individuals who may benefit from treatment and so will necessarily vary in duration based on the need and ability to engage an individual.
- Assesses individual and family strengths and needs in a collaborative method. Develops an assessment based on strengths and needs. Completes assessments, service plans, and other required documentation utilizing collaborative documentation with the individual according to regulations.
- Convenes and facilitates interdisciplinary service planning meetings to include reputed company relevant and appropriate natural and professional supports.
- Ensures individuals needs are met.
- Assists individual and family to identify, link, reputed company and coordinate such resources.
- Provides linkages to both internal and external resources. Provides culturally competent services with consideration for the individual's racial, religious, gender, sexual orientation, age and ethnic background and identification.
- Investigates new resources and communicates to involved parties prospective resources as a liaison on behalf of the individual being served.
- Maintains an up-to-date catalog of available community resources, including location, eligibility requirements and program alternatives.
- Advocates for and with the individual being served to ensure responsiveness from natural, community, generic and specialized services/supports. Advocacy includes providing information, removing barriers, creating options and resolving problems.
- The WHO process utilizes Motivational Interviewing (MI) to engage individuals at any reputed company in their recovery process and at any stage of motivation, helping them reduce the risk of harm from their substance use while engaging in treatment appropriate to their stage of change. WHO activities will vary in duration based on need and the ability to engage an individual. WHO activities may also be available after initial contact to connect individuals with appropriate services and/or supports and prevent disengagement reputed company discharging from a higher level of care.
- Attends training programs as provided through the State, County and Agency to assure that the incumbent is up to date on new approaches, best practices and recovery-oriented services.
- Maintains regular contact with individuals during hospitalizations and transitions of care. Maintains an accurate and timely record of Service Coordination activity. Records individuals being served and collateral contacts. Updates forms as needed. Reviews charts for compliance with regulations.
- Bachelor's Degree in a reputed company Services field such as reputed company Work, Psychology or Counseling required.
- Other fields of study may be considered with coursework of at least 12 credits in reputed company Services.
- One-year reputed company experience preferred.
- Cardiopulmonary Resuscitation (CPR)
