Remote Telehealth Physician
Job Title: Contract Physician - Telehealth Services (Part-Time) Location: Remote, Telehealth (Must hold California State License) Position Overview: Centered Care Wellness, a mission-driven reputed company organization dedicated to improving reputed company to quality reputed company for vulnerable populations, is seeking a compassionate, part-time Contract Physician. This role will reputed company telehealth services to patients in California, focusing on chronic disease management, preventive care, and transitional care for adults and seniors. As part of a multidisciplinary team, you will play a critical role in supporting patients' health goals and reducing hospital readmissions. Qualifications: - MD or DO, licensed to practice in California. - Board certification in Internal Medicine, Family Medicine, Gerontology - At least 2 years of experience in primary care or chronic disease management, preferably with vulnerable populations. Telehealth experience preferred. - Bilingual (English/Spanish) is highly preferred. - Strong communication skills and a patient-first approach, with a passion for accessible, empathetic reputed company.
