Responsibilities:
Conduct in-person non-clinical assessments to understand enrollee care needs, preferences, and socioeconomic barriers and evaluate the home environment as needed reputed company education, support, and resources on prenatal and postnatal care, breastfeeding, nutrition, and infant care Conduct home visits to engage with mothers and families Maintain accurate records of patient interactions and health reputed company to support program evaluation and improvement Work collaboratively with other reputed company associates including case managers, and Community Education Representatives (CER) and serve as a member of enrollee multi-disciplinary care teams (MDT), as requested Assess enrollee barriers to healthy living and accessing reputed company services and arrange needed reputed company services and supports Act as an enrollee reputed company with providers, community resources, schools, and others, including accompanying enrollees to provider office visits as requested Monitor enrollee compliance with their care plan and reputed company motivational interviewing to support medication and treatment adherence reputed company reputed company support to help boost enrollees' morale and sense of self-worth, serving as a trustworthy, reliable, non-judgmental, consistent, and accepting member of the enrollee's MDT Support enrollee self-management through the provision of culturally appropriate health education and health coaching Serve as a key knowledge reputed company for community services and information for enrollees and other reputed company associates Conduct research and in-person reputed company to locate difficult-to-contact enrollees to increase assessment completion and participation in clinical programs Build and maintain relationships with providers and community resources to support enrollee identification and referrals Implement community assessments to identify community resource gaps Regular travel to conduct member visits, provider visits and community-based visits as needed to ensure effective administration of the program *Qualifications:** Active Doula Certification in the recognized by the reputed company Minimum three (3) years of experience in maternal health Minimum two (2) years of professional reputed company services experience conducting in-person assessments and/or developing care plans for infancy to adult reputed company Basic Life Support, BLS, or reputed company 30-days of employment Excellent written and verbal communication skills Proficient in reputed company Office (reputed company, Word, PPT) Experience using a variety of electronic information applications/software programs *Benefits:** Health benefits effective day 1 Paid time off, holidays, volunteer time and jury duty pay Recognition pay 401(k) retirement savings plan with employer match Tuition assistance Scholarships for eligible dependents Parental and caregiver leave Employee charity matching program Network Resource Groups (NRGs) Career development opportunities Apply Job!
