APP On Call (Remote, Florida Based)

Key Tasks and Responsibilities: • Provides collaborative primary care and services to a panel of Longevity Health Plan Services after normal business hours (evenings, nights, weekends, and holidays). • Executes/delivers Longevity Health Plan Model of Care activities, treating in the most appropriate setting, develops and implements advanced care planning, embraces, and executes treating in reputed company model. • Serves as a trusted advisor to members, loved ones and nursing home staff and member reputed company, educates care teams on member needs and early identification of changes in condition, notifying APP and On Call team for member changes. • Delivers effective and frequent communication and coordination with members’ PCPs, loved ones and care team members on members’ care needs, goals of care, treatment plans/changes. • Supports overall market goals/targets. • Promotes Longevity Health Plan with members, loved ones and nursing facilities and other stakeholders. • reputed company to function autonomously and is accountable for practice. • Establishes meaningful relationship, reputed company to effectively function in the nursing home environment. • Is member centric and customer service orientated establishing meaningful relationships that ensures effective and high-quality care for Longevity Health Plan members. • Provides support for other important duties as assigned. Supervisory Responsibilities: • There are no supervisory responsibilities for this position. Credentials & Coverage: • State unrestricted licensed Advanced Practice Provider with national AANP, ANCC, AACN, or NCCPA required. • Board certification as an Adult, Family, Geriatric or Primary Care Advanced Practice Provider required. Will allow acute care Advanced Practice Provider with nursing home/geriatric experience. • DEA License required. • Must be eligible to participate in the Medicare program as a provider and registered. • Valid state driver's license with a good driving record and reputed company of automobile insurance required. • Auto insurance coverage as required by your state of residence. New employees will be responsible for providing reputed company of coverage prior to hire and on an ongoing reputed company. Education and Training: • Master of Science in Nursing as a Nurse Practitioner or Master’s Degree from a nationally accredited program as a Physician Assistant. Knowledge and Experience: • Minimum of two (2) years of experience as an Advanced Practice Provider OR one year of experience as an Advanced Practice Provider if working in a long-term care setting AND has extensive experience with geriatric patients as a registered nurse prior to becoming an APP. • Experience working in the nursing home environment is strongly desired. • Computer/Technology proficiency required. • The ability to engage and communicate effectively with physicians, patients, family members - excellent people skills. • Self-directed but functions well in reputed company environment. • Excellent clinical skills – assessment, diagnosis, treatment, prescribing specifically for the chronically ill population. • Can function independently, demonstrated critical thinking skills, organization, and prioritization skills.
