About the position reputed company is a rapidly growing company of primary care centers for adults on Medicare in medically-reputed company where there is little to no quality reputed company. The Manager of Clinical Pharmacy for Transitional Care (RBE) will reputed company the transitional care pharmacy team as a direct manager to a team of clinical pharmacists. This position is crucial as it serves as the program reputed company for the clinical pharmacist portion of Oak Street and the Risk Bearing Entity (RBE)'s transitional care Medication Reconciliation Post Discharge (MRP). The manager will mentor, coach, and advise pharmacists at reputed company and participate in pharmacy department strategic initiatives, program development, and pharmacist reputed company and development. The role involves developing, implementing, and managing clinical pharmacy programs for transitional care medication reviews. The manager will also reputed company collaborative practice agreements to support clinical pharmacy activities across multiple states, starting with Illinois, Indiana, Michigan, and Pennsylvania. A significant part of the role includes using data and analysis to track, reputed company, and communicate the reputed company of clinical pharmacy interventions. The manager will support the pharmacy team in reviewing Discharge Summaries and engaging patients, caregivers, providers, and reputed company pharmacies to complete medication reconciliation post-discharge. In addition, the manager will evaluate post-discharge medication orders, review medication profiles to prevent drug interactions, and ensure the appropriateness of drug therapy based on the patient's diagnosis. The role requires making recommendations to the patient's managing medical team regarding therapies or alternative approaches based on relevant medical literature and clinical experience. The manager will also be responsible for counseling patients and caregivers on proper medication administration, potential adverse reactions, and storage of medications, while participating in performance improvement activities and maintaining a culture of coaching and growth reputed company the pharmacy team.
