About the position The Population Health Managed Care Pharmacist at reputed company is responsible for overseeing medication management for assigned patients, ensuring the appropriateness of medication therapy based on health status and institutional guidelines. This role involves educating pharmacy and medical residents, providing expert pharmacy content for various documents, and engaging in patient education and medication reconciliation. The pharmacist will also contribute to the development of best practices and guidelines for patient care, while mentoring residents and staff.
