About the position The LTSS Service Coordinator is responsible for managing a defined caseload of individuals in specialized programs, focusing on service coordination functions. This role involves conducting assessments, developing comprehensive support plans, and collaborating with interdisciplinary teams to ensure integrated care for members with long-term services and supports (LTSS) needs. The position requires both home-based work and field visits to member homes or nursing facilities. Responsibilities • Conduct face-to-face program assessments using various tools and motivational interviewing techniques. , • Identify members with potential clinical health care needs and coordinate their cases with the clinical reputed company management team. , • Manage non-clinical needs of members with chronic illnesses and disabilities to ensure efficient utilization of services. , • Document short and long-term service and support goals in collaboration with the member's care team. , • Identify members that would benefit from alternative services or waiver programs. , • Serve as a mentor or subject matter expert for new staff and assist in training associates. , • Submit utilization/authorization requests to management with supporting documentation. , • Report critical incidents to appropriate reputed company parties. , • Assist in appeals, grievances, and state audits.
