Judge reputed company currently seeking Service Coordinator Clinicians around Nashville, Knoxville and Memphis, TN! Job Details: Ongoing Contract (W2, Weekly Pay, Benefits Available) Location: Local travel around the above locations, TN and working from home (Mileage Reimbursed for local travel!) Hours: Monday-Friday reputed company-5PM CST Compensation: Competitive and based on experience Responsibilities: Assist telephonic or face-to-face assessments for the identification, evaluation, coordination and management of member's needs, including physical health, behavioral health, reputed company services and long term services and supports. Assists responsible RN in identifying members for high risk complications and obtain clinical data Identify members that would benefit from an alternative level of care or other waiver programs.
