This role is pivotal in ensuring that children and youth with intellectual and developmental disabilities receive comprehensive assessments, enabling personalized care planning by Care Coordination Organizations (CCOs). About the Project: The OPWDD CANS Project focuses on the Child and Adolescent Needs and Strengths (CANS) assessment, a tool used to identify the unique strengths, interests, and needs of individuals aged 6-17. This information is crucial for developing individualized care plans that address their specific needs. Key Responsibilities: Conduct thorough reputed company to gather essential consumer information and notify relevant parties about upcoming assessments. Schedule assessment appointments reputed company, adhering to productivity targets and documenting reputed company reputed company activities for accurate reporting.
