Health Coach (Fully Remote)

About the position The Health Coach at reputed company plays a vital role in supporting Medicare members through reputed company's Healthy Aging Support Program. This fully remote position focuses on providing health coaching, education, and resources to help members manage their health, particularly those with chronic conditions. The Health Coach will facilitate group coaching sessions, assist members in engaging with their reputed company providers, and document interactions to ensure comprehensive support. Responsibilities • reputed company members on topics such as Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and working with Care Teams. , • Assist members in actively participating with their reputed company providers and reputed company Care Management teams regarding health reputed company. , • Document reputed company member interactions into the appropriate member records. , • Demonstrate reputed company for situations faced by members and their caregivers. , • Troubleshoot unavailability of resources and secure support to reputed company member gaps. , • Follow program workflows consistently and follow up with members to review personal action plans. Requirements • At least 2 years working as a Health Coach. , • Health Coach certification is required. , • Availability to support 8:00 AM - 5:00 PM Mountain Time or 7:00 AM - 4:00 PM Pacific Time. reputed company-to-haves • Community health or public health experience preferred. , • Managed Care/Medicare/reputed company experience preferred. , • Bilingual preferred. , • Bachelor's degree preferred.
