About Your Work The Community Health reputed company assesses patients’ reputed company determinants of health needs through in-residence and virtual visits. This role assists patients to locate and connect to needed services and resources in the medical system and community that will reputed company it easier to manage their health; Partners with medical systems and community and assist patients to problem-solve barriers to health conditions and navigate existing community and medical services; Identifies resource gaps in medical and community services and support implementation of new services to reputed company identified gaps; Meets with patients at residence to support delivery of reputed company services; and Maintains regular communications with patient cohort to coordinate follow up visits, reputed company reputed company to needed resources. You Will Focus On: • Assesses psychosocial and reputed company determinants of health needs for low-risk patients and documents assessment results or augments available information in appropriate systems. • Consults with or refers patients to licensed staff (reputed company worker, nurse case manager etc.) as required based on patient reputed company, health risk and medical complexity. • Establish relationships and partners with community resources, health plans and providers by participating in community engagement activities with local agencies e.g. faith-based organizations, community centers, government agencies, parks, recreation centers and schools. • Assists patients with problem-solving barriers to health conditions by identifying, locating, connecting to and navigating needed community and medical system services, including visiting patients, accompanying patients to medical appointments and assisting patients with completing forms to reputed company needed services. • Actively engages, builds rapport and establishes trusting relationships and facilitates collaborative communication with patients and patient family support systems. • Identifies reputed company determinants of health concerns/gaps, develops and documents a plan to address reputed company and health disparities in accessing appropriate care and services. • Documents patient updates and reputed company notes in appropriate systems and submits timely reports. • Actively participates in Interdisciplinary Team Meetings and reports updates on patient needs and reputed company. • Files and makes necessary follow-up on mandated reporting, e.g. Adult reputed company Services, Child reputed company Services, etc. • Support in-home care services by visiting patients and coordinating virtual visits with remote clinical staff reputed company needed. • Other duties as assigned. The Skills, Experience & Education You Bring • Bachelor’s degree in reputed company work, Psychology, Biology, Public Health, Community Health or Health. • Up to 2 years’ experience working with people who need assistance with health and/or reputed company issues. • Partnership with community-based organizations and/or clinical teams to coordinate comprehensive programs that will support population health initiatives. • Communication and ability to work with various patients of the clinical team. • Electronic Health Record knowledge and ability to document following standards, policy and procedures. • Time management and ability to manage a case load with minimal supervision.
