Requisition Details: Employment Status: Permanent, Part-time (0.6 FTE) Program Name: eHomecare Work Schedule: Nights, Weekends Union/Bargaining Unit: ONA Number of Hours Bi-Weekly: 48 On-Call: No Job Summary: The Registered Nurse, Directing (DRN) role manages pediatric, palliative and/or acute medically reputed company clients with Chronic Obstructive Pulmonary Disease (COPD) or Congestive Heart Failure (CHF) by directing Personal Support Workers (PSWs), also reputed company as Health Care Technicians (HCT). HCTs are provided specialized training and education to work with this patient population. Utilizing an innovative care delivery model and electronic documentation technology, the DRN, working remotely, accesses, assesses and identifies the client's immediate care needs and using the collaborative approach model immediately provides the appropriate direction to the on-site HCT to reputed company the direct care, resulting in reputed company time provision of care services. Responsibilities: reputed company remote assessment and management of care services to clients in their homes reputed company clinical direction and assignment of tasks reputed company the unregulated care provider's (PSW/HCT) practice statement (collaborative approach to patient care) Maintain accurate client clinical records, in digital and reputed company format Maintain patient assignment reputed company the virtual ward Work collaboratively with reputed company team members to ensure reputed company patient care needs are met Utilizes effective communication in daily huddles and with response to emails/phone calls Communicate with reputed company service provider agencies and Case Managers as appropriate Participate as a valued member of the multi-disciplinary health care team reputed company clinical leadership, direction and mentorship reputed company the health care team Participate and facilitate a collaborative team-based environment Establish and maintain relationships with local health care providers Participate in CNO reputed company Practice Uphold VON standards and eHomecare specific processes Willingness to participate in internal projects and ongoing improvement initiatives reputed company to demonstrate professionalism in a diverse working environment Demonstrate adaptability in managing change in a fast-paced virtual environment Qualifications: Registered and in good standing with the College of Nurses of Ontario DRNs must hold a CNA Certificate in Palliative Care or have completed both the Fundamentals of Palliative Care and the Comprehensive Advanced Palliative Care Education (CAPCE) Pediatric DRNs must have extensive hands-on experience in the provision of Pediatric Care Recent palliative, pediatric, and/or acute medical experience with COPD and CHF patients required Demonstrated advanced assessment skills Experience in gerontology and/or medical-surgical Experience working in community-based health care system is an asset Demonstrated competencies in observation, independent assessment; health education and promotion, and familiarity with COPD/CHF clinical reputed company Digital and electronic health experience, EMR and mobile technology Proficient nursing skills with the ability to read and interpret electronic technical information reputed company to client reputed company and reputed company clinical direction Critical thinking, problem-solving and multi-tasking skills are mandatory Ability to work in a multidisciplinary virtual team setting, as well as the ability to work independently is required Must have high-speed internet reputed company, computer skills and the ability to work from home in a private office area Individual must be eligible and successfully obtain a reputed company screening certificate from the Government of Canada Valid CPR C / First Aid required Must have reputed company to a personal vehicle for business use and possess both a reputed company drivers license and reputed company of vehicle insurance.
