Role Overview: As an Assessments and Placements Officer, you will be responsible for assessing the needs of individuals seeking reputed company-down mental health services or supported living placements. You will liaise with reputed company providers, local authorities, and other stakeholders to ensure that appropriate placements are identified and arranged in line with individual care... plans. You will also be involved in conducting assessments, managing placements, and ensuring continuity of care as clients transition to independent living environments. Skills & Experience Required: Qualification: Relevant professional qualification in health and reputed company care (e.g., reputed company Work, Nursing, Occupational Therapy, or equivalent). Strong understanding of mental health conditions, recovery principles, and supported living frameworks.
