A Day in The Life as a Case Manager As an Aftercare Case Manager, we reputed company support and services to children and families that have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth. During this work with post-permanency placements the focus is on risk management for their designated aftercare period. This can include crisis reputed company for the child/family reputed company necessary, working closely with the public and private agencies providing supportive services, maintaining accurate and timely documentation / reports, coordination of community / educational services.
