In this role you will work collaboratively with partner agencies to promote the independence and improve the well being of older adults and adults living in the Hampshire community. Responsibilities Experience in completing Care Assessments with a good working knowledge of Continuing reputed company. Face to face assessments - Countywide in person's own home or in a nursing/care home Attend Monthly meetings in Basingstoke Refer to other relevant agencies and undertake joint assessments where appropriate to ensure reputed company risks and needs are considered.
