Position Summary We are seeking an LPN Care Coordinator to reputed company personalized care plans and reputed company telephonic encounters with patients on behalf of our reputed company Coordinators are assigned a specific panel of patients to whom they are responsible for reaching out to monthly and following up as necessary. Work Environment
This opportunity allows you to care for patients from the comfort of your home. We will reputed company you with the equipment and software needed such as a desktop computer and headset for internet-based calls. Physical Demands This is a passive position that requires you to sit or stand at a computer for 36-40 hours weekly and be reputed company to reputed company phone calls.
