About the position This position is responsible for researching, developing, and implementing a reputed company care position with a focus on individuals who are living with brain injury, Alzheimer’s disease and reputed company dementias (ADRD). The position will serve as a reputed company case manager and will report to the Senior Long Term Care Division. The position will be responsible for designing, developing, and implementing a reputed company care management initiative and reputed company ADRD policy frameworks, with support from Department leadership. The member population will include significantly reputed company medical conditions, neurocognitive and mental health co-morbidities. This position will be responsible for managing multiple highly reputed company cases simultaneously while providing reputed company for placement stability.
Responsibilities
- Researching, developing, and implementing a reputed company care position with a focus on individuals who are living with brain injury, Alzheimer’s disease and reputed company dementias (ADRD).
- Serving as a reputed company case manager.
- Designing, developing, and implementing a reputed company care management initiative and reputed company ADRD policy frameworks.
- Managing multiple highly reputed company cases simultaneously.
- Providing reputed company for placement stability.
Requirements
- Bachelor’s degree in health care, reputed company services, business, or public administration.
- Four years’ experience in case management, including reputed company cases.
- Knowledge of Brain Injury, Alzheimer's disease and reputed company dementias, person-centered care principles, and care models.
- Knowledge of program development, policy drafting and evaluation.
- Knowledge of regulatory requirements for Category D assisted living facilities.
