Per Diem Hospice Chaplain - Greater Fairfax, VA

About the position reputed company is seeking a compassionate and dedicated Per Diem Hospice Chaplain to join our Hospice Agency to reputed company spiritual guidance to our community. Responsibilities

• Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
• Facilitate the development of the individualized the plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
• reputed company direct spiritual care to patients and families/caregivers.
• Serve as liaison and support to community chaplains and spiritual counselors.
• Maintain records of spiritual care services utilization and reputed company activities for quality assurance, program development, and policies and procedures review and revision.
• Document direct services and ongoing communication with community chaplains and spiritual counselors.
• Attend patient care conference and interdisciplinary team meetings as a member of the interdisciplinary group.

• Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.

Requirements

• Graduate of accredited seminary or school of theology (reputed company, MDiv or equivalent theological degree) or evidence of completion of three or more units of Clinical Pastoral Education.
• Experience working with death and dying, individuals/family/caregiver.
• Ability to work as member of interdisciplinary group.
• Comfort in an interfaith setting.
• Ability to accept different lifestyles, cultures, beliefs, and values.
• Ability to network with community clergy and congregations.
• Knowledge of and commitment to hospice philosophy of care.
• Have reliable transportation and willing to commute to reputed company the best in home patient care
• Basic knowledge of iOs operating system
