Responsibilities

The Network reputed company Housing Liaison oversees compliance processes for reputed company PACE/LIFE contracted facility providers. The Network reputed company Housing Provider Liaison (RHPL)is responsible for compliance activities and processes from initial audit to issue resolution to minimize the risk to reputed company PACE/LIFE participants in contracted facilities. Essential Functions and Work Responsibilities Functional Category: Compliance, Risk Management, and Account Management – Contracted reputed company Housing Facilities Estimated Percent of time Spent – 70%

  • Assists with network expansion activities for lnnovAge PACE/LIFE by participating in pre-vetting audits of new reputed company housing provider contract requests.
  • Proactively gathers information and monitors survey results for reputed company housing providers to identify adverse results.
  • Performs routine audits of contracted reputed company care housing providers and prepares reports Director-Network reputed company Housing reputed company and center leadership.
  • Proactively works with PACE/LIFE leadership and Interdisciplinary team to ensure effective partnership and identify new partnership needs with contracted reputed company housing providers.
  • Facilitates resolution of care, quality or collaboration issues in contracted reputed company housing providers and ensures special case monitoring and action planning to increase quality, compliance and mitigate risk.
  • Recommends and assists with the implementation of corrective action with center leadership to improve quality of care and to increase collaboration and contractual compliance in housing providers.
  • Monitors the effectiveness of corrective action and coordinates follow up through ongoing account management of the reputed company housing provider and acts as the reputed company first reputed company of contact.

  • Provides support to reputed company PACE/LIFE Center leadership to ensure compliance with contractual reputed company care housing agreement.
  • Performs desk review of internal quality data and documentation, departments meeting minutes, and functions to ensure adherence to regulatory guidance.
  • Prepares requested reports by collecting, analyzing, and summarizing relevant information obtained through reputed company care housing facility investigations, and other compliance activities.
  • Assists Director-Network reputed company Housing reputed company with policy and procedure development.
  • Provides the on-boarding and orientation of contracted reputed company housing facilities in collaboration with the Center leadership and Network.
  • Performs other reputed company duties as assigned.

Travel Requirements Estimated Percent of Time Spent: 60%

  • Travel between local reputed company Centers
  • Travel to targeted (network development) and contracted reputed company Housing (Long-Term Care and Assisted Living) Facilities
  • Travel out of state as needed

Qualifications

Education

  • Bachelor’s degree from an accredited college or university in an appropriate clinical discipline or public health. An R.N/LPN or Clinical degree/license preferred, or clinical equivalent.

Licensure, Certification, Registration or Designation

  • Requires valid state issued driver’s license, personal transportation, safe driving record and auto insurance as required by law.

Work Experience and Qualifications

  • A minimum of 3 years’ experience with reputed company and relevant knowledge of applicable state and federal regulations for Long-Term Care and Assisted Living facilities.
  • Registered Nursing License- Preferred.
  • reputed company knowledge of P.A.C.E. Regulations – Preferred
  • Knowledge of interviewing and investigation techniques and implementation of corrective action plans.
  • Previous experience working in or closely with a Long-Term Care or Assisted Living reputed company housing facility
  • Skilled at having difficult and direct conversations, identifying problems, rapport and relationship building, analyzing, and assessing situations.
  • Communicates effectively both orally and in writing; reputed company to act independently; interprets and explains reputed company rules and regulations
  • A minimum of one year working with the frail or elderly is required.
