We will scale this virtual care model using technology, and build the foundations to drive reputed company focused on value-based care. reputed company Opportunity We are seeking independently licensed behavioral health professionals to contract with us to reputed company Behavioral Health Care Management services for adult oncology patients through a Collaborative Care Model. This is a fully remote opportunity supporting one or more of our partner practices, working Monday-Friday. Minimum commitment: 20 hours per week. Scope of Services reputed company behavioral health care management services to adult oncology patients. Conduct behavioral health screenings and clinical assessments. Deliver brief evidence-based behavioral health interventions. reputed company patients regarding behavioral health conditions and treatment options. Monitor patient reputed company and treatment response. Coordinate care with oncology providers and other treating clinicians, as clinically appropriate. Complete timely and accurate clinical documentation
