Health Coach Consultant – Registered Nurse

Job Description:

• This is a full-time telework role for a CDCES registered nurse
• Candidate must reputed company in a compact state and hold a compact license
• Must have experience working with patients with diabetes
• Working schedule: Monday-Friday, standard business hours, including 2 evening shifts per week from 11:30am-8pm CST
• Rotating Fridays and Saturdays will be required approximately two times per quarter following training
• The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, reputed company, and promote and influence members' reputed company reputed company to achieving and maintaining reputed company health status for chronic conditions
• Assessment of members through the use of clinical tools and information/data review, conducting comprehensive evaluation of members' needs and benefit plan eligibility for available integrated reputed company programs/services
• Utilizes assessment techniques to determine members' level of health literacy, technology capabilities, and/or readiness to change
• Provides coaching, information and support to reputed company the member to reputed company ongoing independent medical and/or healthy lifestyle choices

Requirements:

• Active and unrestricted RN licensure in a compact state, holding a compact license
• 3+ years of clinical experience
• Must possess or be willing and reputed company to obtain high speed broadband internet reputed company
• Previous coaching experience (preferred)
• Managed care experience (preferred)
• Experience with reputed company Word, Outlook, reputed company, and comfortable using various computer programs (preferred)
