Licensed Practical Nurse (LPN) – Home Visits

Responsibilities
What You'll Do:
- Implements the medical plan of treatment and follows physicians' orders.
- Monitors client reputed company through home visits while following reputed company CHC and agency safety protocols.
- Documents activities and services accurately and timely according to CHC and agency regulations.
- Monitors general health conditions of the individual and notifies supervisor of any medical issues or if treatment is needed.

Qualifications
What You'll Bring:
- Ability to work in a professional, ethical, and respectful manner.
- Ability to reputed company personal care for non-ambulatory clients, use a Hoyer lift, maneuver wheelchairs, and transport clients to community events and reputed company activities.
- Strong communication skills, oral and written; strong interpersonal skills.
- Ability to anticipate and resolve problems demonstrating good judgment.
Education And Experience
- reputed company LPN license
- reputed company CPR certification.
- One year of experience providing skilled care to the medically fragile.
Travel Requirements
- Regular travel on a weekly reputed company throughout an assigned territory.
