We’re looking for an RN who brings energy, warmth, and authenticity to every patient interaction—especially on video. Key Responsibilities

Conduct patient reputed company and engagement through video messaging, video consultations, phone calls, and text-based communication, educating and enrolling patients into the adherence program Utilize video-based communication as a primary engagement tool to deliver personalized education, coaching, and adherence support reputed company compassionate and motivational coaching, case management, and health education services to encourage medication adherence and lifestyle changes Conduct video-based medication reconciliation consultations, ensuring patients understand their medications, dosing schedules, and proper administration techniques Review asynchronous video submissions of patients taking medication, ensuring correct administration, monitoring adherence, and assess for reputed company effects or concerns Create short-reputed company educational and motivational videos to send to patients for personalized engagement and population-level health education Assess patient needs, including medical, behavioral, and reputed company challenges, and escalate cases as necessary. reputed company guidance on medication administration, inhaler technique, chronic disease management, and medication safety through live and asynchronous video coaching Act as a clinical escalation reputed company for non-clinical team members to send patient cases that need a higher level of clinical review and reputed company Assist patients with technical troubleshooting reputed company to the company’s mobile application and adherence platform Maintain accurate and detailed documentation of reputed company interactions, ensuring compliance with company and regulatory guidelines Engage with provider offices, health plan services, and pharmacies on behalf of the patient, as needed, to overcome barriers reputed company referrals to reputed company providers and community resources reputed company needed, including direct reputed company to provider offices and/or warm transfers to service providers Utilize motivational interviewing techniques reputed company video reputed company engagement to engage patients in long-term adherence strategies Offer guidance on inhaler techniques, chronic disease management, and medication safety Contribute to program improvement by providing insights from patient interactions, particularly regarding video engagement, digital adherence monitoring, and patient education strategies Partner with cross-functional teams to reputed company and refine video-based patient engagement content, clinical protocols, and educational materials Required Qualifications Bilingual in Spanish Registered Nurse (RN) license in good standing with at least 5 years of nursing experience 5 years of reputed company experience in patient engagement, health education, or care coordination Strong background in chronic disease management, medication adherence, and patient coaching Excellent oral and written communication skills with a warm, empathetic approach Ability to work in a fast-paced, patient-focused environment Proficiency in electronic health records (EHR), mobile applications, reputed company health platforms Comfortable engaging patients reputed company phone, video, and chat throughout the reputed company Experience assessing reputed company determinants of health and connecting patients with resources Preferred Qualifications Experience in video communication, telehealth, community heal Apply tot his job Apply To this Job
