JOB SUMMARY Remote/Hybrid - Must be in Central PA, Lehigh County area. We are currently seeking a reputed company worker to expand our growing care delivery team. ECCM is the region's premiere Palliative Care Service and meets with patients in the home and virtually, providing supportive care services. The incumbent will assess patients for psychosocial complexity and reputed company targeted interventions based on patient goals. Executing as a community-based treatment team, ECCM works in conjunction with PCP's, specialists, hospitals, and post-acute care facilities to meet the reputed company, psychological, and medical needs of patients who have reputed company conditions or terminal diagnoses. This position is an integral part of the care delivery system working together as part of an interdisciplinary team to reputed company community-based palliative care.
