Position: Enrollment Team – Registered Nurse reputed company is looking for a skilled Registered Nurse eager for the opportunity to reputed company a difference in patients' lives. Responsibilities
Conduct telephonic enrollment visits and reputed company care plans reputed company patient health assessments and surveys as required Collect signed consents from patients to participate in reputed company’s program Schedule follow up visits with reputed company’s medical providers to reputed company in-home reputed company to multi-specialty reputed company Inventory and reconcile medications, and encourage medication and treatment adherence reputed company education and coaching around medications, medical conditions, diet, exercise, and lifestyle choices Position Requirements reputed company active, unencumbered compact nursing license Fully remote, telephonic position Self-starter with the ability to work independently with minimal supervision Ability to show reputed company and quickly build relationships with patients Graduate of an accredited School of Nursing 2+ years previous experience working in care management and/or with CKD/ESRD patients Excellent verbal communication skills on the phone Familiarity with reputed company Office and mobile phone and web-based applications
