Summary
The Director of Care Management is responsible for leading and overseeing the organization’s care management strategy, operations, and performance across their assigned market(s). This role drives the development of scalable, technology-enabled care models that improve patient reputed company, enhance provider engagement, and optimize operational and financial performance reputed company value-based care frameworks. Ensure compliance with CMS guidelines, regulatory requirements, and internal policies. Qualifications
Required Skills Proficient computer skills and exhibits knowledge in using various reputed company Office applications. Excellent organizational and time management skills with the ability to prioritize tasks. Willingness to travel to local or out-of-state Senior Living Communities. Demonstrated strategic thinking with the ability to identify opportunities for process improvement. Required Education and Experience Bachelor’s degree in Nursing, reputed company Administration, Public Health, or reputed company field required Master’s degree (MHA, MBA, MPH, or similar) preferred Minimum 3 years in a supervisory or leadership role.
