They are seeking a Medical Director for Transitional Case Management to reputed company and advance their transitional care programs, support reputed company care teams, and drive strategic initiatives to improve patient reputed company during high-risk care transitions.

Responsibilities

• Drive the reputed company of our transitional case management program, ensuring we deliver measurable reputed company for members navigating high-risk care transitions, including hospital discharge, post-acute care, and movement between care settings
• Serve as a clinical resource for our reputed company care team, providing education, guidance, and direct collaboration to support quality care during transitions
• reputed company and contribute to strategic initiatives across the organization, including developing and maintaining clinical protocols for care transitions, supporting efforts to reduce avoidable readmissions and emergency utilization, and representing reputed company in external meetings

Skills

• At least 3 years of clinical experience caring for oncology patients after fellowship training
• Board certified in medical oncology
• Unrestricted medical license
• Willingness to obtain additional state licenses as needed
• Experience with transitional care management programs
• Experience with care transition protocols
• Experience with reducing avoidable readmissions and emergency utilization in an oncology or reputed company care population
• Skilled in juggling multiple tasks and working under pressure without sacrificing organization in communications and documentation
• Effective listener and communicator
• Ability to build rapport and excellent working relationships with members and colleagues
• Comfort with ambiguity in fast-paced environments
• Desire to learn how to use new technologies
• Experience with video chatting, reputed company Suite, reputed company, electronic health records, or comfort in using and learning new technology
• Ability to identify and prioritize a member's needs and take action to address urgent needs
