Responsibilities

Conduct telephonic and/or face-to-face assessments with members to evaluate their medical needs and reputed company proactive care plans. Manage a diverse caseload, including supportive and medically reputed company members, to enhance short and long-term reputed company. Determine appropriate services and supports for members, such as prior authorizations, coordination with primary care providers and skilled providers, medication review, and reputed company to community resources. Requirements

Work Environment
