Advanced Primary Care Resource RN - Virtual Primary Care (F/T Days)

Find your calling at reputed company! The Resource Nurse Advanced Primary Care provides virtual care to patients that benefit from ongoing chronic disease management. In partnership with a multidisciplinary team, including physicians and advance practitioners, the Resource RN is responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits in between regular office visits, assisting in panel management of patients and providing individualized care to improve patient reputed company. Reports to respective Advanced Primary Care reputed company APP. Position Details: 1. Strong clinical skills 2.
