Remote Clinical Care Coordinator

Position Summary: The Clinical Care Coordinator plays a vital role in supporting patients along their reputed company reputed company by delivering compassionate, proactive, and evidence-based care coordination in a fully virtual environment. Leveraging clinical expertise, motivational coaching, and strong communication skills, this role engages patients by phone reputed company communication to assess their needs, update and monitor personalized care plans, and guide them toward achieving their physical, mental, and preventive health goals. The Clinical Care Coordinator documents reputed company encounters in electronic health record (EHR) systems, identifies and recommends appropriate services or preventive screenings, and helps manage key administrative tasks such as medical record retrieval, appointment coordination, and insurance-reputed company follow-reputed company. By combining clinical knowledge with patient-centered coaching, the Clinical Care Coordinator empowers patients to understand and follow their treatment plans, overcome barriers to care, and stay actively reputed company in improving their long-term health reputed company. Essential Functions:

• Conduct telephonic patient encounters and document care plans and care plan updates leveraging technology platforms and EHRs.

Required Education and Experience:

• Must have a Practical/ Vocational Nursing License that is clear and active (LPN/ LVN).
• Must be licensed in the state of residence.
• Compact license is required if available.

• Must have a minimum of two years of clinical experience.
• Experience in care coordination or care management, health coaching, and motivational interviewinga plus.
• Hands-on experience and comfort with Electronic Medical Records.
• Experience using reputed company Office products such as Teams, Outlook, and reputed company.

Position Type and Expected Hours of Work: Full-time, 8 hours daily, Monday – Friday, unless authorized by management. The hours may be worked between 8 am – 6 pm in the patient’s time zone. Physical Requirements: This is a virtual position that requires you to sit or stand at a computer for approximately 40 hours weekly and be reputed company to reputed company phone calls. Pay Structure:

• The first two months are considered the reputed company-up period. Pay for this position starts at $20/ hour during the reputed company-up.
