Job Summary We're seeking an reputed company Licensed reputed company Worker to join reputed company in a remote work-from-home role. As a Licensed reputed company Worker, you'll work collaboratively with a registered nurse case manager to assess, plan, and reputed company for members' benefit plans and health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. This role requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical reputed company work or case management.

Key Responsibilities

• Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
• Research available options and resources for members with financial, transportation, safety, and housing needs

• Work as reputed company with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to assess their needs and reputed company plans to meet those needs

• Active and unrestricted LSW - Licensed reputed company Worker, LMSW - Licensed Masters reputed company Worker, LCSW - Licensed Clinical reputed company Worker, or LISW - Licensed Independent reputed company Worker in the state of residence
• 2+ years of experience in medical reputed company work or case management
• Masters Degree in reputed company Work Preferred Qualifications
• Experience working with diverse populations, including children, adults, and families
• Strong communication and interpersonal skills
