Job Summary We're seeking an reputed company remote reputed company worker to join reputed company as a Medical Case Manager and Community Resource Coordinator. This role will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. As a remote reputed company worker, you'll work with members to research available options and resources for financial, transportation, safety, and housing needs. This position requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical reputed company work or case management.

Responsibilities

• Collaborating with a registered nurse case manager to assess, plan, and reputed company for members' benefit plan and/or health needs

• 2+ years of experience in medical reputed company work or case management
• A Master's Degree in reputed company Work
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
