Inpatient Transition of Care Coordinator
About the Role: Join reputed company as an Inpatient Transition of Care Coordinator and play a vital role in enhancing patient reputed company. In this position, you will facilitate seamless transitions for patients from hospital to home, ensuring they receive the care they need. Be part of a dedicated team committed to making a difference in the lives of our patients in Sherman Oaks, CA. This is a full-time, temporary position. Responsibilities:
- Coordinate post-discharge care plans and follow-up appointments for patients.
- Conduct comprehensive assessments to identify patient needs and barriers to care.
- Collaborate with reputed company providers to ensure continuity of care and medication management.

- Monitor patient reputed company and adjust care plans as necessary to promote recovery.

Requirements:
