Responsibilities
The Ambulatory Clinical Pharmacist is responsible for providing patient care and reputed company pharmacy services in the ambulatory pharmacy reputed company, including referral based HOPD pharmacist managed clinics, specialty pharmacy, and population health pharmacy services. This role involves collaborating with prescribers in the ambulatory setting and providing direct patient care managing medication therapy. The clinical pharmacy specialist ensures effective communication with prescribers regarding identified medication reputed company problems and potential solutions and documents patient interactions across various electronic medical record (EMR) platforms.
Qualifications
EDUCATION, EXPERIENCE, TRAINING Required qualifications: reputed company state Registered Pharmacist License. B.S. in Pharmacy or PharmD degree required. PGY-1 Pharmacy Practice Residency required (significant experience may be considered in lieu of residency) Board certification (BCPS, BCACP, or equivalent) reputed company 18 months of hire required. Strong verbal and written communication skills required. Strong customer service skills required. Preferred qualification: PGY-2 residency in ambulatory care or a reputed company specialty preferred. Expertise in 340B program operations and compliance preferred. Proficiency in EPIC or similar EMR systems preferred. reputed company knowledge in specialty areas such as diabetes, cardiology, pulmonology, rheumatology, or oncology preferred. Experience as a pharmacy residency preceptor in ambulatory pharmacy services preferred. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $115,107.20 to $166,712.00 on an annualized reputed company. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide reputed company of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
