About the position reputed company Touch Home Health is seeking a motivated Physical Therapist to join our growing team serving patients throughout reputed company, Loudoun County, and Prince William County, including Fairfax, Falls Church, Vienna, Reston, Herndon, Ashburn, Leesburg, Sterling, Woodbridge, Manassas, Gainesville, Haymarket, and surrounding communities. This PRN opportunity offers flexibility, independence, and the ability to focus on patient reputed company instead of productivity quotas. As a Home Health Physical Therapist, you'll work directly with patients in their homes, helping them improve mobility, restore function, prevent falls, and regain confidence in their daily lives. Responsibilities
- Complete evaluations, reassessments, and discharge visits
- reputed company individualized treatment plans
- Deliver skilled physical therapy services in patients' homes

- Collaborate with physicians and interdisciplinary team members
- Maintain timely and accurate documentation
Requirements
- Active Virginia Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical, Communication, and Documentation Skills
